Flow Chart on Change of Account Name

Step 1:

Inquire at the Customer Service/Complaint Desk the requirements for application and status of the account.

Step 2:

Fill-up and submit Service Application Form C-3 together with other requirements needed to the Customer Service/Complaint Desk for verification.

Step 3:

Pay corresponding fees and charges to the Teller.

Step 4:

Present Official Receipt to the Customer Service/Complaint Desk and sign logbook for the request.
