Flow Chart on Rerouting of Water Service Connection
Step 1:
Inquire at the Customer Service/Complaint Desk the requirements for application and status of the account.
Step 2:
Fill-up and submit the Service Application Form C-4 together with other requirements needed to the Customer Service/Complaint Desk for verification
Step 3:
Pay corresponding fees and charges to the Teller.
Step 4:
Go back to the Customer Service/Complaint Desk and present Official Receipt.
Step 5:
Sign logbook for your service request at the Customer Service/Complaint Desk.
